How our practice works
We function like a doctor’s office except that we come to you. We schedule our visits weekdays, Monday through Friday. There is a doctor or nurse practitioner available by phone 24 hours a day, 7 days a week. Instructions on how to reach us and when to call us will be given at the first visit.

We serve requests for medical home visits for adults who need medical care and are unable to leave their homes without considerable and taxing effort. Referrals from physicians are welcome.
For those eligible, Housecall Providers will bill Medicare or Medicaid and other insurance. However, no one will be denied care due to lack of insurance.

Additional patient criteria for eligibility
Patients must also meet the following criteria:

  • Live in private home, adult foster care home, or assisted living facility.
  • Over the age of 18 (no pediatrics).
  • We accept Medicare, Medicaid, CareOregon and most other insurance.
  • We are on the provider panels of most HMOs.
  • No emergency first visits.

What to expect
We are currently experiencing so many new referrals that we are unable to keep up with them. Most of our clinicians have a waiting list. Please call to find out if there is a clinician serving your zip code who is accepting new patients. Before we can process an application for our services we need to have a complete and accurate Patient Information form.

Once we have accepted a patient, that person will be assigned to one of our clinicians on the following Monday morning. The clinician will contact the patient by phone by Tuesday afternoon to schedule a first visit.

The first visit includes a complete history and physical examination. This can take an hour or more. Please be sure someone is present who can sign forms and help give medical information.

At the first visit we will usually review Advance Directives if these have already been established, and discuss treatment wishes in case of medical emergency. Our goal is to have a signed POLST (Physicians Orders for Life-Sustaining Treatment) for every patient. Visit OHSU to download the OHSU POLST form.

Clinicians visit their patients once per month on average, more often if necessary.

A note about hospital visits
We do not provide in-patient hospital care, but work with the hospitalists at most area hospitals to insure continuity of care. Except for dire emergencies, we request that patients contact us prior to going to a hospital so that we can pave the way.
