Complex problems require well-coordinated solutions, and for residents of Washington County, the Collaborative Older Adult Solutions Team (COAST) is working to identify and implement solutions to even the most difficult situations.
COAST works with adults, their families, and support systems to develop and facilitate solutions to complex problems by bringing together a variety of resources across multiple systems. COAST meets monthly to review and discuss referrals for their specialized consultation. This interdisciplinary, team-based approach allows for effective partnerships and an efficient response time.
Examples of COAST team members include the Washington County Sherriff Department Mental Health Response Team, Washington County Disability Aging Veteran Services, Tuality Geriatric Psychiatry, and behavioral health provider, LifeWorks Northwest.
Housecall Providers primary care provider, Ivan Wang PA, has a patient who was recently referred to COAST and was grateful for their expert assistance. “Wonderfully collaborative!” Ivan reports, “Finding and mobilizing the depth and variety of resources provided to our patient would have required hours of work were they not organized together as COAST.”
