Our highly experienced and qualified staff of physicians, nurse practitioners and physician assistants provides primary medical care exclusively in the homes of nearly 1,400 patients throughout the Portland metropolitan area. Meet our team.
We Look at the Big Picture
Treating patients in their homes help our clinicians practice better medicine because they can see the whole picture at a glance – living conditions, the functional status of their patients and the quality of care they are receiving.
Holistic and Coordinated Care
Our primary care providers (PCP) tailor care plans to fit patients’ needs, life circumstances and treatment wishes. With the support of care coordinators in the office, our patients get connected with the services they need such as in home blood draws, x-rays, EKGs, referrals to specialists – whatever the PCP thinks is needed. Our proactive coordinated care prevents emergency room visits and hospitalizations.
The transition team assists the patient’s primary care provider in preventing unnecessary emergency room visits or hospitalizations. When a caregiver notifies us of a health crisis requiring a trip to the hospital, our team becomes the patient’s advocate throughout the entire hospitalization and discharge process.
