Who is Housecall Providers?

Housecall Providers is a nonprofit healthcare organization dedicated to caring for people whose health makes it difficult to leave home. For more than 30 years, we have served individuals whose medical needs, mobility challenges, or life circumstances make traditional clinic-based care hard to access, bringing compassionate, high-quality medical care directly to where people live.

Founded in Portland in 1995, Housecall Providers was built on a simple belief: health care works best when it is personal, relational, and grounded in an understanding of the whole person. That belief continues to guide our work today.

Care that says with you

Housecall Providers is one organization offering three integrated, in-home medical programs, primary care, community-based palliative care, and hospice, designed to support patients over time, not just at a single moment of need.

As health needs change, our care adapts. Patients and families are able to remain connected to an organization they already know and trust, supported by care teams who understand their medical history, personal goals, and lived experience. Rather than asking people to start over with new providers when care becomes more complex, we strive to remain present walking alongside patients and families as circumstances evolve.

Who we serve

Our patients are adults of all ages, most often living with multiple chronic conditions and complex medical needs. Many are homebound, medically fragile, or facing serious chronic illnesses. Some live in private homes; others live in assisted living or adult care homes. All deserve care that honors their dignity, values and choices.

We recognize that health is shaped by more than diagnoses alone. Our teams take time to understand each person’s environment, support system, and priorities, providing care that reflects the realities of daily life.

How we care

In-home medical care at Housecall Providers is delivered through provider-led, interdisciplinary teams that may include physicians, nurse practitioners, physician associates, nurses, social workers, spiritual care providers, pharmacists, care coordinators, schedulers and volunteers. Working together, these teams focus on improving quality of life, reducing unnecessary hospitalizations and supporting both patients and those who care for them.

Our approach emphasizes listening, collaboration, and shared decision-making. Care plans are shaped by what matters most to each patient, whether that means managing symptoms, maintaining independence, or finding comfort and peace at the end of life.

Housecall Providers is deeply rooted in the communities we serve because we were created by our community. As a nonprofit organization, our focus is not on volume or profit, but on access, equity and long-term impact. Over the decades, our work has demonstrated that home-based care can improve health outcomes, increase patient comfort, reduce health care costs and contribute to care team satisfaction.

Supported by our community

As a nonprofit organization, Housecall Providers exists because of the generosity and trust of our community partners. Philanthropic support helps ensure that people with serious chronic illness, limited mobility, or complex needs can receive compassionate, in-home medical care regardless of circumstance.

Community support allows us to respond to unmet needs, invest in innovative programs, and provide services that are not fully covered by traditional reimbursement. It strengthens our ability to care for the whole person and to remain present for patients and families when care becomes more complex.
